Preventing Falls Among Older Adults
Falls threaten older adult safety and independence, but falling does not need to be an inevitable result of aging. By putting prevention measures in place, the number of falls among older adults can be significantly reduced.
The Challenge
As loved ones age, the chances of becoming injured from a fall increase. In fact, the National Council on Aging found that:
- 1/3 of Americans aged 65+ fall each year.
- Every 13 seconds, an older adult is treated in the emergency room for a fall; every 20 minutes, an older adult dies from a fall.
- Falls are the leading cause of fatal injury and the most common cause of nonfatal trauma-related hospital admissions among older adults.

- Hygiene & Toileting: Accessing the need for grabbars, shower chairs/transfer benches, easy to access shower head, toilet riser, etc.
- Grab Bars: Verifying grab bars are installed properly and in high-risk areas.
- Transfers: Creating a plan for moving from one spot to another.
- Entrances & Pathways: Make sure you have a clear path for getting in/out and removing tripping hazards.

Continuous Care Planning
Prior to discharge, Westchester Family Care will schedule a training session with the client and caregiver. This will help us maintain consistency in transfers and exercise while providing the client with an opportunity to regain a level of independence.
Once home, a formalized care plan will be developed and monitored through supervisory RN visits. This will help us track compliance and progress through documentation to ensure we are providing the best services, as promised!
